BEFORE THE MOBILITY <\/div><\/span>\n\t\t\t\t\t\t\t\n\t\t\t<\/i><\/span>\n\t\t\t<\/i><\/span>\n\t\t<\/span>\n\n\t\t\t\t\t\t<\/summary>\n\t\t\t\t

1. Actively search for information about a University you would like to apply<\/strong> to, and also look for its courses and activities\u2019 schedule<\/strong> which are available for Erasmus+ students. The best source to do this are websites of the Partner Universities.<\/b> <\/p>

2. During the recruitment, prepare an application form for studies <\/b>and then submit it to Erasmus+ Coordinator IN YOUR DEPARTMENT. Then the coordinator will submit your application to the Erasmus+ Office.<\/b><\/p>

3. The Erasmus+ office will send an official nomination for your stay to a Partner University you have chosen.<\/p>

4. Wait for an e-mail from the Partner University<\/strong> concerning further steps of your application.<\/p>

5. Follow all the required application procedures.<\/strong><\/p>

As soon as possible, send the documents to the Partner University. They are as follows:<\/p>

\u2022 APPLICATION FORM*<\/p>

\u2022 ONLINE LEARNING AGREEMENT*;<\/p>

\u2022 APPLICATION FOR ACCOMODATION (if applicable)**<\/p>

\u2022 OTHER DOCUMENTS REQUIRED BY THE PARTNER UNIVERSITY*<\/p>

*(the application procedures may vary depending on the university you apply to. Thus, you should carefully read all the instructions and complete the requirements given by the Partner University)<\/strong><\/p>

**Students arrange their accommodation during Erasmus mobility themselves (in dormitories \u2013 with the help of the Partner University\u2019s Student Service Office, or on their own \u2013 letting agencies, other sources). Most of universities have strict deadlines regarding sending those documents <\/strong> \u2013 please, check them in advance on Partner University\u2019s websites.<\/p>

6. Prepare (with your Program Coordinator<\/b>) an Online<\/strong> Learning Agreement<\/b> before the mobility. Gain all the required acceptations \u2013 yours, your Program Coordinator\u2019s, the Partner University\u2019s.<\/p>

7. Start a visa application process<\/strong> (if needed)<\/strong>. The Erasmus+ Office will prepare and give you an official document confirming your Erasmus+ recruitment that will allow you to obtain a visa. Later, you will also get a grant for your stay. <\/p>

8. Deliver a copy of your health insurance valid in the country of your stay to the Erasmus+ Office.<\/strong> <\/p>

Also, bring: <\/b>\u2013 the STATEMENT ABOUT THE FORM OF GRANT PAYMENT <\/b>(IMPORTANT<\/b>: fill in the declaration with your bank account number KEEPING THE GAPS BETWEEN THE NUMBERS<\/b>, the declarations without the gaps will not be accepted) – CO-FINANCING STATEMENT <\/b> – STATEMENT ABOUT THE PREVIOUS MOBILITIES <\/b>WITHIN THE FRAMEWORK OF ERASMUS+ PROGRAMME and STATEMENT ABOUT COMPLETED PERIODOF STUDIES<\/b><\/p>

9. In case of a situation you write a thesis<\/strong> (Bachelor\u2019s, Master\u2019s or Engineering) during your Erasmus+ mobility, you have to obtain an acceptance from your thesis supervisor<\/strong> (e-mail or paper form).<\/p>

10. If you are a beneficiary of a social scholarship<\/strong>, you are allowed to apply for an extra grant of 250 Euros. <\/strong>To do so, you will need a copy of your social scholarship decision (VALID).<\/p>

11. You sign a contract with the Universty of Opole \u2013 approximately 2-3 weeks before the mobility<\/strong>. Go to the Erasmus+ Office with all the required Signing the contract is compulsory to take part in the Erasmus+ mobility and get a grant. We advise you to arrange the exact date IN ADVANCE, BY TELEPHONE. Thanks to that, signing the contract will be conducted easily, effectively and without extra waiting. <\/p>

12. After signing the contract in our office the first part of your grant will be registered to be paid to you. You will be also given a a licence for language support (the access to an OLS platform) where you will be able o learn the language of your mobility. <\/p>

1. After arriving at the Partner University come to its Erasmus+ Office\/International Relations Office and report your arrival. You will be provided with the most important details of your stay there.<\/div>

2. Within 4-5 weeks prepare the changes in the Online Learning Agreement During the Mobility (LA)<\/strong> \u2013 with your Program Coordinator. The document has to be accepted by your Program Coordinator and the Partner University.<\/div>

3. Before the departure you will be provided with:<\/div>
\u2022 TRANSCRIPT OF RECORDS <\/strong><\/div>
\u2022 CONFIRMATION OF STAY<\/strong>\u2013 MPORTANT! the dates must comply with the dates on the contract signed with the University of Opole! We advise you to visit the International Relations Office in your Partner University, this document (original version) must be provided to our office (Erasmus+) later on.<\/div>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/details>\n\t\t\t\t\t\t

1. Within 60 days after coming back<\/strong> to the University of Opole (but no later than the 10th of September) come to the Erasmus+ office and provide: <\/p>

\u2022 TRANSCRIPT OF RECORDS<\/strong> <\/p>

2. Within 30 days<\/strong> from departure fill in:<\/p>

\u2022 STUDENT QUESTIONNAIRE <\/strong>\u2013 do it on-line, the login and password details will be sent to you in a separate a-mail<\/p>

2. Please, send the copy of the transcript of records to your Program Coordinator and the Dean\u2019s Office and put the grades obtained during the mobility in the USOS system. If they are any differences in study programmes, you have to complete them at the University of Opole after your return. <\/strong>.<\/p>

3. The Program Coordinator accepts your grades in the USOS system and the mobility is then officialy closed.<\/p>
